According to the regulations of Ministry of Health, first time patients who visit our Institution have to bring one of the following documents:
- I/C
- Passport
- Child Birth Cert
For infertility consultation, the patients must be below the age of 40 and married no less than two years.
For oncology consultation, the patients must bring the medical report.
